Job Description
As a Social Worker II, you will provide a wide range of social services to support patients and families in their mental health journey.
Your responsibilities include:
- Conducting psychosocial assessments.
- Delivering resource-based interventions and counseling.
- Providing case management, discharge planning, and risk screening.
- Leading prevention, promotion, and public awareness initiatives.
- Collaborating with interdisciplinary team members and community agencies to enhance patient care outcomes.

Key Responsibilities
Patient Care and Support
- Conduct individual, group, and family counseling.
- Assess patient needs through comprehensive psychosocial evaluations.
- Plan and manage patient transitions and discharge strategies.
Interdisciplinary Collaboration
- Work with a team of healthcare professionals to design patient care plans.
- Coordinate with community agencies for additional patient resources.
Prevention and Awareness

Ideal Candidate Profile
Skills and Qualifications
- Bachelor’s degree in Social Work.
- Active or eligible registration with the Alberta College of Social Workers (ACSW).
- Strong interpersonal, analytical, and problem-solving skills.
Experience
- Previous experience in mental health services.
- Preferred experience in acute care settings as a social worker.
